SweatSonic Inc. Medical Disclaimer and Release of Liability Agreement

1. Acknowledgment of Risk and Release of Liability: I, the undersigned, understand that

participating in any exercise program, diet plan, or use of any fitness equipment provided by

Sweatsonic Inc. involves risks of injury, up to and including death. I hereby acknowledge these

risks and agree to release, indemnify, and hold harmless Sweatsonic Inc., its affiliates, agents,

employees, and any other associated personnel from any and all claims, demands, or causes of

action arising from my participation in any activities or use of any equipment provided by

Sweatsonic Inc.

2. No Substitute for Professional Medical Advice: The guidance and resources provided by

Sweatsonic Inc. are for informational purposes only and are not a substitute for the professional

medical advice, diagnosis, or treatment from a qualified healthcare provider. I agree that I will

not disregard or delay seeking professional medical advice based on anything I read on

Sweatsonic Inc.’s website or materials.

3. User's Health Responsibility: I confirm that I am voluntarily participating in these activities

and using equipment provided by Sweatsonic Inc. I warrant that I have sought medical advice

from a healthcare professional regarding my participation in Sweatsonic Inc.’s programs or use

of equipment, or I have decided to participate without seeking such advice and thus assume all

responsibility for my participation.

4. Physical Condition and Limitations: I declare that I am physically fit and have no medical

condition that would prevent my full participation in Sweatsonic Inc.’s fitness programs. I will

notify Sweatsonic Inc. of any medical conditions, disabilities, or physical limitations that may

affect my participation in their fitness programs.

5. Age and Capacity Confirmation: I affirm that I am of legal age (18 years or older) and possess

the legal capacity to enter into this agreement. If not, a parent or legal guardian has reviewed

and agreed to this disclaimer on my behalf.

6. No Guarantees of Outcomes: I acknowledge that individual results from participating in

Sweatsonic Inc.’s programs may vary and that Sweatsonic Inc. makes no guarantees regarding

specific outcomes or results.

7. Insurance: I represent that I have adequate health insurance to cover any injury or damage I

may suffer while participating in Sweatsonic Inc.’s programs, or I agree to bear the costs of such

injury or damage myself.

9. Emergency Procedures: In case of a medical emergency, I will immediately contact emergency

10. Agreement to Terms: By participating in any program or using any equipment from
